GLP-1 Care Without Having to Audition for It

Why so many patients feel judged or dismissed asking about GLP-1 and metabolic care, and what precision, judgment-free management actually looks like.

An obesity medicine specialist put it about as plainly as it can be put. "Obesity medications are the only ones that we make our patients audition for," Dr. Ali Zentner told the Globe and Mail. Nobody has to prove they deserve a blood pressure medication or an antibiotic. But ask a primary care doctor about GLP-1 medication for weight or metabolic health, and it's common to be met with suspicion first and clinical evaluation second, if the conversation gets that far at all. NPR reported one primary care doctor's actual response to a patient asking about it: "Have you tried Noom? Or meals from HelloFresh?"

This isn't a fringe request anymore

Per J.P. Morgan Research, roughly 25 million Americans are projected to be on GLP-1 treatment by 2030, up from about 10 million in 2025, 6 million in 2024, and 5 million in 2023. This is one of the fastest-growing areas in medicine, and it's growing because the science behind metabolic and weight management has genuinely changed. What hasn't caught up is how much time and seriousness most doctors are able to give the conversation.

Part of the friction is real and worth naming honestly. Even when a physician believes GLP-1 therapy is the right approach, insurance often insists on step therapy, trying cheaper alternatives first, before it will cover the medication patients actually need. Per the 2025 KFF Employer Health Benefits Survey, 57% of large employers still don't cover GLP-1 medications for weight loss at all. I want to be direct about something here. Why this is a whole-person conversation, not a prescription pad

Weight and metabolic health rarely exist in isolation.
